Cyl 5 misfiring, dealer proposes tear down, Extded warranty fighting to not cover

All started with check engine lite on and car misfiring. Error codes say cyl 5 and 7 coils were bad. Swapped coils and no luck, tried new plugs and this worked for about 50 miles and then back to misfire but only on #5.

Took it to the dealer who has tried all new coils, new plugs, injectors, then tried some programming (something with valve adjustment), compression test and leak test. Dealer says next steps are to tear down and see if one of the cams is bad and if it's not that replacing the entire control module for the car. He's pretty frustrated they can't figure it out. They think it's fixed and then drive it and as soon as it's under heavy load it starts misfiring again.

Extended warranty (warrantech ultimate coverage - which is the highest coverage they offer) says they won't cover the tear down if it doesn't result in a proven failure, i.e. if it ends up being the control module. This is after they sent an inspector out to check things out (and agree on next steps or authorize tear down, so I thought, evidently extended warranties don't seem to operate this way).

1st time with this deal, BMW Fairfax. I had such lousy luck with BMW sterling and I had heard Fairfax was better so I asked for the most experienced tech they had. Car only has 46K on it. Seems really odd to have a cam go bad.
Real frustration is this was the primary reason I bought the extended warranty for. Claim they only pay for troubleshooting for failed parts:) It appears that's much of how that crazy industry works.
